Job Description

The Keswick Community Health Associate will support the delivery of all service line programs including Communities for Life (CFL), Housing Upgrades to Benefit Seniors (HUBS), Home & Healthy (HH), and Wise & Well Education. Under the direction of the Senior Director, this associate will be familiar with daily operations and responsibilities including but not limited to: service navigation, membership customer service, class preparation and facilitation of wellness education. This role will support clients through the coordination of wrap around services and connection to community resources that enable the community member to live well at home for as long as possible.

ESSENTIAL JOB DUTIES

  • Assist with support of CFL case load to engage with members on a regular basis.
  • Conduct member needs assessments and guide them in utilizing their benefits to support their health and wellness goals.
  • Stay abreast of community resources and best practices in health and wellness resources, senior home repair, care transition and person-centered care.
  • For Home & Healthy clients, identify and initiate referrals for social service and other programs, including financial, psychosocial, community and supportive services in support of an overall care plan.
  • Document all service coordination efforts and outcomes and update as needed in the appropriate database.
  • Network with other community agencies to make them aware of Keswick's services and learn of their services.
  • Deliver evidenced-based programs, health and wellness classes and activities (in-person, virtually, and at off-site locations).
  • Assist the day-to-day operations and programs in response to facilitator needs, room set-up and take-down, technology needs, attendance, and reporting.
  • Support membership associate duties including phone coverage, class registration, prospect tours, & outreach events.
  • Identify opportunities and facilitate referrals across Keswick service lines.
  • Other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIREMENTS

  • Strong coordination and administrative skills.
  • Familiarity with resources of interest to older people in Baltimore City.
  • Ability to work well with older adults to help them achieve their health and wellness goals.
  • Knowledge of Baltimore City neighborhoods and businesses.
  • Knowledge of AD/ADRD and best practices to serve those clients and care partners during all stages of the condition.
  • Excellent interpersonal skills. Ability to work well with individuals from a wide variety of ethnic, racial, linguistic, educational, and socio-economic backgrounds.
  • Effective communication skills, oral and written.
  • Attention to detail and strong problem-solving skills.
  • Proven ability to work on multiple projects and deadlines and to work independently as well as in a team.

EDUCATIONAL AND WORK EXPERIENCE REQUIREMENTS

  • AA degree required; Bachelor's degree preferred in gerontology, aging services or a closely related human services field.
  • 1+ years related work experience with older adults and/or service navigation preferred.
  • 1+ years related work experience with persons with dementia/cognitive impairment.
  • Experience delivering programs live and virtually in multiple settings and using different media platforms, preferred.
  • Experience facilitating evidenced-based programs preferred.

QUALIFICATIONS

  • Able to pass criminal background check.
  • Current driver's license, dependable licensed and insured vehicle.
  • Covid19 vaccination.
  • Local travel required. In addition to office-based and telephonic responsibilities, the Community Health Associate may spend approximately 10% of scheduled hours in the field visiting clients/partners in the community when necessary.
  • Proficient use of computer software (Excel, Word, Outlook, Zoom, etc.)
  • Ability to lift 50 lbs. occasionally.
