Job Summary

Under general supervision, uses a client-centered or motivational approach to behavior change and proven intervention models to assist clients recently diagnosed with a chronic illness or clients with prior diagnosis and currently out of care. Provides basic care coordination, identifies and reduces barriers to care, and provides referrals to necessary resources based upon individualized client needs and circumstances. Actively interacts with clients in community, clinic, or correctional care settings.

Essential Functions:
  • Manages assigned caseload in community, clinic, and correctional care settings for clients recently diagnosed with a chronic illness or clients identified with prior diagnosis and currently out of care. (45%)
  • Assures linkage to medical care and evaluates and refers clients for other needs including on-going case management, social support, and psychiatric and substance abuse care. (30%)
  • Uses tracking systems to document client encounters, outcomes, assessments, follow-ups, and contacts with other agencies. (15%)
  • Participates in the preparation and presentation of Linkage to Care data and program results. (5%)
  • For Patient Care Positions: * Ensures all duties, responsibilities and competencies are conducted in a manner that is effective and appropriate to patients/clients to whom care/service is being provided. * Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient's status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided. * Employee has completed and met their clinical competency standards. (5%)

Education:
  • High School Diploma or GED Required

Work Experience:
  • 1-3 years Typical candidate will have at least one year case management experience working with high risk populations for HIV/STDs Required or
  • 1-3 years Experience in another area of social services and case management. Required

Licenses:

    Knowledge, Skills and Abilities:
    • Ability to use Microsoft Office.
    • Competency in assessment of client's medical, mental health, and psychosocial needs.
    • Knowledge of available community resources for medical, psychological, and social needs.
    • Knowledge of infectious disease transmission, care, and treatment.
    • Basic competency in motivational interviewing
